Please sign this waiver for all children’s under 18 years old
In Consideration of being allowed to enter the play area and/or make usage of any equipment of Little World PLAY/ Playtown, myself, my spouse, my children, and any other children i am responsible for the day identified below, acknowledge, appreciate and agree to the following conditions:
1. I release, indemnify and discharge Little World Play/ Playtown owners, agents, officers, affiliates, volunteers, participants, employees, and all other person or entities acting in any capacity on their behalf.
2. I represent that I am the parent or legal guardian of the participant(s) named above, or I have obtained permission from the parent/legal guardian of the participant(s) named above to execute this agreement on their behalf. I agree that the participant(s) named above and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any party and/or program offered by Little World Play.
3. I expressly agree and promise to accept and assume all of the risks existing in this activity. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks, both known and unknown including those that may arise out of the negligence of other participants.
4. I acknowledge that the rental made entitles children. I agree to supervise all children invited and not leave them unattended nor without the supervision of a parent (s) or guardian at all time.
5. I recognize that some of the equipment provided for children might be dangerous or result in an accident if not used properly.
6. I will share all playground rules with parents/ guardian and guarantee that all children follow instructions.
7. I agree that all children interactions and activities made within the playground are under my own risk.
8. I understand that if a child is not following all the instructions/ rules of the playtown and/ or damaging the equipment consciously, he/ she would need to be removed.
9. I hereby voluntarily release, forever discharge and agree to indemnify and hold harmless Little World Play/ Playtown from all claims, demands, or causes of action, which are in any way connected with my participation and the children in the playground activities and equipment including any such claims with allege negligent acts or omissions of Little World Play/ Playtown.
10. Should Little World Play/ Playtown or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
11. I certify that I and all parents/ guardians have adequate insurance to cover any injury or damage the children may suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical condition I may have.
12. I grant Little World Play/ Playtown the right to photograph and/ or videotape me and my children and use my and/ or my children’s name, face, likeness, voice and appearance in connection with exhibitions, publicity, advertising and promotional materials without reservation or limitation.
13. I allow Little World Play/ Playtown to add my email to their email list to receive latest LWP information, offers, packages and future add on.
By signing this document, I acknowledge that if anyone is hurt or property is damaged during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against Little World Play on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by these terms.
